In Vitro Fertilization for Infertility

In vitro fertilization (IVF) is an assisted reproductive technology used for the couple who are unable to conceive or having difficulty in carrying a healthy pregnancy.  During IVF procedure an egg and sperm is brought together in the laboratory conditions for fertilization process.

For in vitro fertilization (IVF) either a donor egg or a donor sperm can also be used for fertilization. After the fertilization process the fertilized egg is generally placed in the uterus for further development.

IVF Treatment Process

The common steps involved in IVF process include:

1.      Ovulation and Egg Retrieval

Ovulation needs to be controlled with hormone therapy for an assisted reproductive procedure. Hormone treatment is required to prevent unpredictable ovulation which can lead to cancellation of an in vitro attempt. Hormone treatment is done by using two similar kind of gonadotropin-releasing hormone analogue that comprises of GnRH agonist and GnRH antagonist.

During this procedure you will first receive GnRh agonist for around 10 days to shut down your pituitary glands. You will recieve ovary stimulating hormone on a daily basis. After the administration of hormones, your doctor will closely monitor for 2 weeks before the egg retrieval.

You can also use gonadotropin or follicle stimulating hormone (FSH) injections at home to make your ovaries produce multiple eggs.  You can start FSH injection in the initial stages and then take GnRH antagonist injection after 5 days that will help in inhibiting the production of luteinizing hormone (LH) within few hours.

Your blood estrogen levels are checked after the hormone treatment. Ultrasound treatment is used to determine whether the eggs have matured in your follicles. Ultrasound treatment and blood tests are done after the first week. During the second week the hormonal dosage may vary depending on the test results. Transvaginal ultrasound and certain blood tests are done after the second week of treatment.

When the follicles are fully developed, human chronic gonadotropin hormone (hCG) injections are given to stimulate your follicles till it gets matured.  The mature eggs are usually collected after 34-36 hours through needle aspirations guided by ultrasound. During this procedure certain pain relieving medicines and sedation medicines are given for quick relief from pain.

2.      Sperm Collection >

Sperms are generally collected by masturbating. In rare cases the sperms are collected from the testicle by making a small incision. Incisions are made to collect sperms, when you experience a blockage that prevents the sperm from being ejaculated or when the sperms are not fully developed. Frozen sperms can also be used and thawed on the day when the eggs are collected and ready for fertilization.

3.     Fertilization and Embryo Transfer

For fertilization, the eggs and sperms are placed in a glass Petri dish and incubated at a suitable temperature to prevent it from infections. Incubation is done for about 48 to 120 hours. After 2-5 days of fertilization, the best fertilized egg is selected. Around 2-3 fertilized eggs are placed in the uterus using a tiny catheter that is inserted through cervix. The remaining fertilized eggs can be stored through cryopreservation process for future use.

What to Expect after the in vitro Fertilization Treatment

Complete development and a healthy pregnancy completely depend on the female partner. Regular blood tests and monitoring is required when super ovulation is carried out by using hormone injections. Mild cramps are experienced by the patients during the procedure and your doctor may advice your to avoid strenuous activities depending on your health condition.
